Level 2
Coordination of Supports
We assist participants to connect and implement their NDIS plans. We enable them to access continued support and build their capacity and understanding of the NDIS. Our Support Coordinators work collaboratively with their clients to ensure their choice and control is upheld and goals are met.
Level 3
Specialist Support Coordination
Provided by one of our registered clinicians, this support is targeted to more complex cases and will be stated as such in a client’s NDIS plan. In addition to the roles outlined in Level 2 support, Level 3 support includes management of crisis situations and ongoing monitoring of these circumstances in the long term where necessary. This may mean linking in with broader support networks to overcome immediate and significant barriers to plan implementation.
